Every chapter! Please complete this form to the
Council Secretary for the
upcoming year, regardless of the installation date, by 1 January each year and
any changes thereafter during the year as necessary. NOTE: If there
are no changes you may so indicate on this form by filling in your
chapter name, and number and checking the box "No Changes" or contact the Council Secretary.
After completing the form, be sure to click on either SUBMIT button. The
changes are sent to the Secretary & Web
Coordinator by e-mail.
